Some insurance plans require that you receive a referral from your primary care doctor before seeing a specialist.  For your convenience we have web links to the insurance carriers we are in network with.  Even though we are in network with these insurance carriers, that does not mean we are in network with every plan that each insurance carriers has.  It is your responsibility to determine who you may see, what your benefits are, and if your insurance plan requires a referral.  Therefore, we strongly suggest you check your benefits and referral requirements prior to making your appointment.  You may check your benefits by either calling the phone number on the back of your insurance card, or checking on-line.  If a referral is not obtained prior to your visit, but is required, you will be responsible for the total cost of your visit at the time of service.
